The Family Support and End of Life Services Team, established in April 2020, is a unique nursing-driven service offered at Ascension St. John Hospital. The team's mission is to honor life, restore dignity in death and ensure each patient receives extraordinary, consistent and individualized end-of-life care through compassionate support and empowerment of patients, families and healthcare workers providing care to them. With your support, we can continue offering innovative and individualized care to the bereaved through evidence-based training, the creation of mementos, holistic modalities, and healing environments.
